FERPA and COPPA Consent Form
Lux Mundi Learning Online Spanish Courses
This Consent Form is required for all students participating in Lux Mundi Learning’s Online Spanish 1 and 2 Courses, in compliance with the Family Educational Rights and Privacy Act (FERPA) and the Children’s Online Privacy Protection Act (COPPA). For students under 18 (FERPA) or under 13 (COPPA), a parent or legal guardian must provide verifiable consent for the collection, use, and disclosure of personally identifiable information (PII) and participation in recorded activities.
1. Collection and Use of Personal Information
Purpose: Lux Mundi Learning collects PII (e.g., student name, email, voice, image) through Canvas, Zoom, VoiceThread, and Lingua Meeting for educational purposes, including coursework, conversation practice, and peer feedback.
FERPA (All Students): PII in education records (e.g., Zoom recordings, VoiceThread submissions) will be shared only with the course instructor, enrolled students in the same course section, and parents/legal guardians upon request, as permitted by FERPA.
COPPA (Students Under 13): For students under 13, PII will only be collected, used, or disclosed with verifiable parental consent. Parents may review, modify, or delete their child’s PII by contacting support@luxmundilearning.com.
2. Consent for Recordings
Zoom Sessions with Lingua Meeting: I consent to the recording of Zoom sessions that may include the student’s name, voice, or image for educational purposes. These recordings will be stored securely and shared only with authorized course participants or parents/legal guardians, as outlined in the Terms and Conditions.
☐ Yes, I consent
☐ No, I do not consent (alternative arrangements will be provided)
VoiceThread Recordings: I consent to the student creating video recordings via VoiceThread, which may include their name, voice, or image, to be shared with the course instructor and enrolled students for educational purposes. I waive any claim to additional rights or compensation for these recordings, provided they are used solely within the course.
☐ Yes, I consent
☐ No, I do not consent (alternative arrangements will be provided)
3. Parental Rights and Revocation
FERPA: Parents/legal guardians of students under 18 may inspect, review, or request amendments to education records by contacting support@luxmundilearning.com.
COPPA: Parents/legal guardians of students under 13 may revoke consent, request deletion of PII, or refuse further collection by contacting support@luxmundilearning.com. Revocation may limit course participation if alternative arrangements are not feasible.
Opt-Out: If you do not consent to recordings, Lux Mundi Learning will provide alternatives (e.g., camera-off participation, alternative assignments), though this may impact participation grades.
4. Third-Party Platforms I understand that Lux Mundi Learning uses third-party platforms (Canvas, Zoom, VoiceThread, Lingua Meeting) that are vetted for FERPA and COPPA compliance. I consent to the student’s use of these platforms for course activities, as described in the Terms and Conditions.
